FINANCIAL ARRANGEMENTS     In order to provide the highest quality care on a sound basis, we have developed a full range of convenient payment options for you to choose from. Prior to the commencement of treatment, fee and financial arrangements will be discussed with you. Our front desk will help you to understand and file your insurance benefits. FEES · PAYMENTS · INSURANCE Before any treatment is rendered; we will consult with you on your present state of health, your challenges, our concerns and objectives, and an estimate of our fees for your necessary services. In order to keep costs passed to you at a minimum, payment at the time of service is requested. Billing accounts may be opened with our financial coordinator in advance of services. Please ask our financial coordinator for details or to discuss an arrangement. Insurance claims can be filed by the office on your behalf or you may file your own claims and be reimbursed directly. For your convenience our office does accept cash, VISA, MasterCard, personal checks, or money orders. A written estimate will be provided for all complex treatment. We will gladly give you a written estimate for any routine procedures upon request. If you have insurance coverage, a pre-determination can be sent and financial arrangements for the balance not covered by insurance can be made when this information is received. A down payment is required for more involved, complex or extended treatment, regardless of insurance coverage or pre-determination of payment by your insurance company.
